Assessments such as patient questionnaires, direct observation, and formal sleep studies (i.e., polysomnography) also may be done. The patient should be questioned about sleep patterns, sleep environment, and sleep hygiene at home to determine whether the insomnia is a new problem or a continuation or exacerbation of an existing problem. If a hospitalized patient complains of insomnia, the physician should obtain a thorough sleep history. 10 Poor sleep hygiene, another factor, includes frequent or lengthy naps during the day. 4,9 A study of 50 hospitalized critical care patients revealed a mean of 43 interactions per patient per night, with 62% of routine baths performed between 11 pm and 6 am. These activities include checking vital signs, bathing, drawing blood, changing dressings, and making room changes. 8Ī lack of diurnal light cycles also can contribute to insomnia, as can clinical activities performed at night. The Environmental Protection Agency recommends a maximum of 45 decibels during the day and 35 decibels at night. 4 One study found the noise level in hospitals to be 50 to 70 decibels during the day and an average of 67 decibels at night. Noise-from staff conversations, alarms, pagers, intercoms, doors, medical equipment, or televisions-is a major contributor. Many studies have found the hospital environment to be a contributing factor to insomnia. 5 Medications associated with insomnia include anticholinergics, antiepileptics, bupropion, beta agonists, caffeine, corticosteroids, methylphenidate, nicotine, selective serotonin reuptake inhibitors, and theophylline. Other medical conditions associated with insomnia are fibromyalgia, dementia, epilepsy, traumatic brain injury, Parkinson's disease, and stroke. Physical factors that can cause sleep problems include pain stress difficulty breathing (e.g., from chronic obstructive pulmonary disease (COPD), pulmonary embolus, heart failure) cough frequent urination and withdrawal from illicit drug or alcohol use. Psychological factors associated with insomnia include anxiety, depression, and grief. 5 These sleep disorders can lead to nighttime awakenings, sleep-onset difficulty, and daytime sleepiness. ![]() 6 PLMD, which is characterized by repetitive flexion and extension of the lower leg during sleep, occurs in about 80% of RLS patients. 6 The estimated prevalence of RLS is 5% to 15%. Usually associated with leg discomfort, RLS is relieved with movement prolonged bed rest during hospitalization can worsen it. RLS is characterized by an urge to move the legs while at rest, especially at night. 4,5 This sleep fragmentation and oxygen desaturation causes snoring or gasping during sleep and can lead to hypertension, cardiac arrhythmias, cognitive decline, and increased mortality. OSA, which occurs in 24% of men and 9% of women in the United States, is characterized by repetitive episodes of breathing pauses (apnea) and hypoventilation (hypopnea) that precipitate brief awakenings. Sleep disorders include obstructive sleep apnea (OSA), restless legs syndrome (RLS), and periodic limb movement disorder (PLMD). Multiple factors can lead to insomnia in hospitalized patients, including sleep disorders, psychological and physical factors, certain medical conditions, medications, environment, clinical activities, and poor sleep hygiene. 1 In a longitudinal observational study of insomnia prevalence in 280 hospitalized elderly patients, 21% reported new-onset insomnia, 38% reported moderate or severe insomnia, and 38% reported sleep disturbances during hospitalization, and severe morbidity, pain, and impaired functional status were significantly associated with insomnia. Interestingly, when the medical records were reviewed, no patient had physician documentation of insomnia symptoms. 2 In one study, 47% of 222 patients reported that they had insomnia and/or excessive daytime somnolence while in the hospital. 1 It is characterized by an increase in sleep latency (trouble falling asleep), a decrease in sleep maintenance (trouble staying asleep), or a decline in sleep quality (less time sleeping compared with time spent in bed). ![]() ![]() It turned out all I needed was a fuel delivery as my fuel gauge was broken so I didn't realize I had run out of gas! The tow driver said he had the auto parts back at the shop and my car was ready the next day. The check engine light wasn't on and I had an auto tune up recently so I thought I might just need a jumpstart or car battery replacement or some kind of fluid delivery but my husband thought it might be a more serious auto repair problem maybe with the auto electrical system? The driver from Highway 30 was helpful and very polite especially when he diagnosed the problem. Apparently not all tow truck companies offer 24 hour towing so I'm glad this company did! I didn't know what was wrong with my vehicle and my husband wasn't much help over the phone. I called Highway 30 for emergency towing and roadside assistance after my car broke down off the interstate on my way to Burley late at night. If I lived anywhere near Twin Falls I would have you maintain my vehicles. This is why your customer service exceeds great. You could have charged me an unreasonable amount of money, but you didn't. You didn't need to make the Maxima a priority, but you did. You could have balked at dealing with me since I was out of town, but you didn't. There are a large number of rare amino acid polymorphisms in the MC1R gene, some of which may also be associated with red hair (Beaumont et al. ![]() Most alleles have only one of these three red-associated amino acids for example, some alleles have cysteine at position 151 but arginine and aspartic acid at positions 160 and 294. This shorthand means that the common amino acids at positions 151, 160 and 294 in the protein are arginine (R), arginine, and aspartic acid (D), while the amino acids cysteine (C), tryptophan (W), and histidine (H) are found in redheads. (1997) identified the three most common amino acid polymorphisms that are associated with red hair: R151C, R160W, and D294H. They found several amino acid variants that were found in red-haired people but rare in non-red people. (1995) surveyed DNA sequence variation in the melanocortin 1 receptor (MC1R) gene. Reed (1952) reviewed the various hypotheses that had been proposed, including that red was recessive that red was dominant that red was dominant, but could be masked by brown or that red was usually recessive but could sometimes be dominant. Davenport and Davenport (1909) found numerous examples of two brown-haired parents having red-haired offspring, which would suggest that it is determined by a recessive allele, but Neel (1943) found 13 out of 114 offspring of two red-haired parents to have non-red hair. The variety of hair colors makes it difficult to summarize the results of family studies in detail. This suggests that when the red pheomelanin pigment is present, whether a person has red hair is determined by the amount of brown eumelanin pigment that the person also has. Reed (1952) also examined hair under a microscope and found the reddish pheomelanin granules that are common in red hair were also in some individuals with brown hair. He found that there was no clear separation of hair into two categories instead there were some individuals with intermediate hair that could not easily be classified as red or non-red. Reed (1952) criticized the subjectiveness of this approach and used a reflectance spectrophotometer to measure the amount of light reflected by hair at different wavelengths. Most studies divide hair color into a small number of categories, such as blond, red, and brown. People with large amounts of pheomelanin have red hair, which can range from pale red ("strawberry blond") to bright red to reddish brown. The amount of eumelanin ranges continuously from very little, producing light-blonde hair, to large amounts, producing black hair. ![]() Hair color is determined by the amount of eumelanin (which is dark brown) and pheomelanin (which is reddish).
